The information gathered from this form will help us promote your business during Seattle Restaurant Week. We want to let Seattle know about your business, a little about your story, and how people can find you! 

Please complete this form within two weeks of your registration. The final deadline to submit is September 15. Please note: We may make edits to your answers and assets as needed for media promotion, clarity, and consistency. Also in this form are fields to upload your business logo and photos.
